By agreeing to this Waiver & Release of Liability (the “Agreement”), you acknowledge that you understand and accept the terms outlined below.

1. Scope of Activities
This Agreement applies to participation in all activities conducted at or in connection with FTY Lab Canada, including but not limited to indoor soccer games, leagues, practices, training sessions, camps, clinics, tournaments, fitness activities, use of facilities and equipment, spectator areas, change rooms, parking areas, and all related programs or events (collectively, the “Activities”).
2. Assumption of Risk
I understand and acknowledge that participation in the Activities involves inherent risks, including but not limited to physical contact, slips, trips, falls, collisions with other participants or equipment, muscle strains, sprains, fractures, concussions, serious injury, illness, or exposure to communicable diseases.
3. Waiver and Release of Liability
To the fullest extent permitted by the laws of the Province of Ontario, I hereby waive, release, and forever discharge FTY Lab Canada, including its owners, directors, officers, employees, contractors, volunteers, agents, sponsors, and affiliates (collectively, the “Released Parties”) from any and all claims, demands, damages, losses, actions, or causes of action arising out of or related to my participation in the Activities, including those resulting from the negligence of the Released Parties.
